Registration & Issuance of new birth certificate
The following documents should be presented for registration and issuance of new birth certificates:
1. Application forms (SH4-1 & SH4-2).
2. The original passports of parents.
3. The original Iranian birth certificate of parents.
4. A full detailed English Birth Certificate (this NOT will be retained).
5. Payment of £10.00 by filling the credit card form or to HSBC Bank, 92 Kensington High Street, Kensington London W8 4SH or postal order.
6. 2 passport size photographs (for children over 2 years old).
Please contact 020-7937 5225 for an appointment after 48 hours of postage. The father should sign the documents when the certificate was issued.
Renewal of old birth certificates
The following documents are required for the renewal of old birth certificates:
1. The relevant application form (SH1).
2. The original old birth certificate including a photocopy of that.
3. 4 (6x4) photographs with personal detail written on the back (ladies with Hejab).
4. Photocopies of pages 2-9 of passport and the page holding visa (2 copies).
5. If the application is made for son/daughter, a complete set of photocopies of his/her passport should be attached.
6. Payment of £11.00 by filling the credit card form or to HSBC Bank, 92 Kensington High Street, Kensington London W8 4SH or postal order.
Duplicate copy for lost birth
certificates
The following documents are required for a duplicate copy of lost birth certificates:
1. The relevant application form (SH2-1).
2. The photocopy of old births certificate (if it is available).
3. 4 (6x4) photographs with personal details written on the back (ladies with Hejab).
4. Photocopies of pages 2-9 of passport and the page holding visa (2 copies).
5. Affidavit forms (SH2-1) (3 witnesses should be available to sign the form at the consulate and if it was not possible, photocopies of witnesses' passport should be enclosed).
6. Payment of £21.00 (for the first copy) and £43.00 (for the second copy) by filling the credit card form or to HSBC Bank, 92 Kensington High Street, Kensington London W8 4SHor postal order.

The time needed, to issue an old birth certificate, depends upon the Iranian organisations, which are dealing with it, and it is not predictable.
